Columbarium Nameplates

Process:

1. Name of decedent year of birth and year of death is provided by the family of the decedent

2. Any member of the Memorial Committee may complete order form provided by Homecoming, Inc. (See attachment #1)

a. Select facing stone option of:

· Single

· Companion

· Shared

 

By Circling one Choice.  Circle companion or shared when the first person has died.

 

b. Print or type name of decedent, year of birth and death

c. Add any comments

d. For Granite choice:  Write Dakota Mahogany

e. For highlight:  Write Almond

f. Fax order form to Homecoming, Inc. per the fax number on the form

g. When companion or shared person is added at a later date, complete a new form including a, b, d, e above, add name of 2nd decedent, year of birth and death.  In comments add, “Send only one urn.”  Fax form to Companion Inc.  A new facing stone with both names will be provided.  The first facing stone will be offered to the family.  If they do not want it, it will be placed in the trash.

3. Urn and facing stone is usually delivered to the church office within five working days.  When 3 or 4 plain facing stones have been removed from the columbarium they should be sent to Companion Inc.

4. Family is contacted for further instructions on placement of the ashes

5. The church secretary has a tool for installing face plates and there is an additional tool in the memorial file cabinet.
